Right MCA inferior division M2 occlusion with complete neurological recovery

Clinical history
A 75-year-old male presented with acute left-sided paralysis with an NIHSS score of 10, last seen well approximately 1.5 hours prior to arrival. Imaging confirmed acute occlusion of the right middle cerebral artery (MCA) inferior division at the M2 segment. Intravenous thrombolysis (IV tPA) was administered promptly within the treatment window.
IV tPA given within the therapeutic window on initial assessment.
Despite thrombolysis, digital subtraction angiography (DSA) revealed the M2 artery remained occluded with no clinical improvement.
Decision made to proceed with endovascular mechanical thrombectomy. The clot was successfully retrieved on the first pass.
Complete recanalization of the right MCA inferior division confirmed on post-procedure imaging.
Complete recovery
The patient achieved complete neurological recovery by the following day and was discharged home after a 3-day hospital stay.
